Synagis and your DSO

It’s “Synagis Season” again for many home infusion companies.  As a clinician it’s “no fun” to see such sick kids and as parents there are financial challenges as well when their insurance coverage is inadequate or unavailable.  The cost and low profit margin on the drug requires that a home infusion company pay attention to this area of their business, during this season.

Consider the impact of a seasonal, high priced service such as providing Synagis on your DSO.  If you are not careful you’ll be caught off guard at the end of the season and here is why. 

Days sales outstanding (DSO) is a calculation used by a company to estimate their average collection period.  The formula to calculate it is your Accounts Receivable balance / average daily sales for a period, usually 90 days.  If your A/R is $1,000,000 and for the last 90 days you’ve generated $1,000,000 in revenue (or $11,111/day), your DSO is 90 days.  The lower your DSO, the faster you are collecting your money and the more bragging rights you have with other providers. 

High revenue seasonal services like Synagis can lower your DSO.  What’s wrong with this?  We want a lower DSO, right?  Absolutely, but you must be careful that you don’t take your eye of the non-Synagis portion of your business.  Let me explain. 

Any time you have a spike in revenue, especially when it is a service like Synagis that you can get paid for rather quickly if you are doing a good job in your billing area, you’ll see your DSO go down.  Why?  Because your sales are going up fast and the money from these sales is coming in quickly.  The reimbursement staff is excited because the company DSO has gone from 73 to 48.  This is great because it usually happens around the end of the year, about 3 months into Synagis season.  The bad news comes when Synagis season is over, the revenue stops and you are left with A/R that is old, A/R that you have not paid attention to as you should have and A/R that is harder to collect.  Spring is turning into summer and your DSO is 94. 

The best solution to this is to manage your DSO by payer, therapy and biller/collector to make sure you see the entire picture and can addresses issues quickly.  Carve out special business situations like Synagis season so you don’t lose focus of the rest of your business.  Use the A/R Analyzer, Revenue Analyzer and Profitability Analyzer reports in Rock-Pond Reports to make sure you see the entire picture and keep your business on course through change.

Submitting a claim by a payer’s timely filing deadline is essential to running your business successfully. If you submit your claim late, the payer will deny payment on it. Rock-Pond can help you avoid this problem with their timely filing reports for HomecareNet and CPR+.

These reports recently made it much simpler for a Rock-Pond customer who uses the CPR+ system to submit claims in a timely manner. Rock-Pond’s timely filing reports gave them a list of timely filing deadlines for their active insurance companies and gave them detailed reports that showed their unbilled tickets and invoices in their A/R with the reason it hadn’t been billed and the amount expected from each claim.

The company’s director of operations said that while CPR+ has some good tools, the Rock-Pond reports she uses gives her greater detail that allow her to group her information how she needs it.

“CPR+ is great because it will highlight in red if something is timing out, but [the Rock-Pond report] will tell you exactly how much time you have and I think that’s so much better,” she said.

Eliminate Timely Filing Errors

Most payers have timely filing limits after which they will deny a claim. Unless you’re willing to work for free, submitting these claims quickly is essential in order to avoid a denial.
 
Rock-Pond has reports available for HomecareNet and CPR+ that will assist with finding claims with deadlines that are in danger of lapsing.
  
Rock-Pond report “Timely Filing Deadlines by Plan within Payer” (!ha_1056) for HomecareNet gives a list alphabetized by payer, the name of the plan and the number of days you have to submit a claim. Rock-Pond’s “Active Insurance Companies by Timely Filing Deadline” (!c2_1329) for CPR+ also lists the timely filing days for each payer and adds the number of unbilled tickets and active patients you have for each. Rock-Pond reports save time by eliminating the need to look up each payer separately in order to determine how close to that payer’s timely filing deadline you are.
With CPR+, you can also run a timely filing alert report that will show either unbilled tickets (!c2_1329a) or invoices in your accounts receivable (!c2_1329b). Both will show the patient, therapy, ticket or invoice number, the filing limit, the number of days left, the reason the claim has not been submitted and the amount each ticket is for. With Rock-Pond reports, you will automatically know which patients or therapies you are having timely billing issues with and whether or not there is a valid reason each claim has not been submitted to the payer.

Therapy Focus to Improve Collections

In last week’s report of the week we talked about managing your A/R by focusing on your HIGH DOLLAR accounts. Another valuable way to review your A/R is by therapy type. In business in general, companies manage their business by product line. For home infusion, therapy type is like a product line. You can run the A/R analyzer report by therapy type to see what therapies are not being paid. Or, run it by therapy within payer to quickly identify problem therapies for specific payers. A more detailed understanding of the situation will almost always lead to a better solution.

The other important point to take from this example is that one Rock-Pond report can often be run many different ways to provide information for different needs. The A/R analyzer allows up to 3 groups where you can select from eight values for each. In the above example you could have the primary group be patient status so you can see all of the old A/R where you are still providing service to the patient!

2011 Rock-Pond reSOLUTIONS

Mediware User Conference

Attending the Healthcare Automation User Conference in Rhode Island is always a highlight of our year and this year was no exception.  It got better this year since we were able to connect with both our HomecareNet and Ascend customers as a result of Mediware’s acquisition of both of these companies.  One on one time with the HomecareNet engineers, reconnecting with customers and friends, meeting new people and of course, Karaoke and the New England Clam Bake all added up to make this a great couple of days.

Rock-Pond unveiled our new product, the Rock-Pond Information Center and announced our partnership with MHA to bring exciting new information tools to the home infusion industry.  More information on these two announcements will appear on our website very soon.  It is an exciting time to be in the home infusion industry and we look forward to taking the excitement and energy from the user conference back to the office.

HomecareNet PECOS tracking

One of the great things about HomecareNet is that it is so robust. Sometimes viewed as complicated, it all pays off when you need to respond to business / industry changes. It’s great for the Mediware development staff too since they don’t have to constantly release “little fixes” with special instructions so the system will handle new, unforseen requirements.

The word of the moment is PECOS, the CMS physician registration process that everyone is scrambling to make sure their referring physicians have completed so they can get paid from Medicare. As a home infusion provider you need to do three things:

  1. Identify the physicians you work with that are not registered.
  2. Change your intake process to verify that the physician is registered with PECOS.
  3. Contact physicians that are not registered to “encourage” them to register or make a decision not to take their patients. 

Do these three things and PECOS will not be a problem for you.  Best of all, these are not hard.  If you are using HomecareNet from Mediware, we recommend:

  1. Use the Rock-Pond PECOS Database Audit to find out exactly where you stand right now.
  2. Set up a PECOS organization in your HomecareNet system and add it as an affiliation to all of the physicians you work with who you’ve verified are registered in PECOS.  Modify your intake process to check this whenever a Medicare patient is admitted. 
  3. Work with your physicians to see that they get registered.

A/R Adjustments – made EASY

It’s bad enough that you have to adjust off accounts receivable. This is money that you have booked as revenue and counted on receiving. In most home infusion companies the adjustment process is a manual process of filling out an adjustment request form, getting it approved by a manager, re-looking up the invoice and applying this adjustment. The end result is a lot of errors, overstated A/R, wasted time by collectors calling on invoices that should have been adjusted, etc.

The manager runs the Detailed Adjustments with Invoice Status report each day to review the adjustments from the prior day and either approve or reverse them with notes in the system to the collector. The results included time savings, improved accuracy, elimination of paper, improvement of audit trails in the system and a much better understanding of why A/R was being adjusted in the first place so it could be prevented.
